SOCIAL SERVICES COORDINATOR, NUTRITION PROGRAM FOR THE ELDERLY DEFINITION:
Under direction, develops, coordinates, and supervises the delivery of a variety of programs designed for the specific health, education, transportation, and recreation requirements of participants in a nutrition project for the elderly; does other related duties as required.
EXAMPLES OF WORK:
Plans, implements, and supervises a project-wide system to deliver and record supportive social services at nutrition meal sites for the elderly which includes outreach, transportation, escort service, shopping assistance, information and referral, nutrition education, recreation, health, and welfare counseling.
Serves as a liaison between community groups, the grantee agency, the area agency, and the project director to effectuate the coordination of related social service programs and to ensure utilization of all available appropriate community resources to participants of the nutrition program.
Evaluates the supportive social services delivered to the project participants and the impact of such services upon the participants.
Coordinates with Site Managers regarding individual participant needs and eligibility for supportive services.
Acts as a consultant at meal sites in the determination of group needs for supportive services.
Recruits, trains, supervises, and evaluates staff persons and volunteers responsible for interviewing participants and the delivery of social services project-wide.
Assures contract compliance on reporting requirements for social services through coordination of data from all sites, and maintenance of adequate files.
Develops and maintains an accurate, up-to-date information bank on agencies, services, training aids and personnel resources to maximize social services project-wide.
REQUIREMENTS:
NOTE: Applicants must meet one of the following or a combination of both experience and education. Thirty (30) semester hour credits are equal to one (1) year of relevant experience.
Six (6) years of professional experience in the field of social service to the aged or in a community-oriented program, either public or private, which deals with elderly people.
OR
Possession of a bachelor's degree from an accredited college or university; and two (2) years of the above-mentioned professional experience.
OR
Possession of a master's degree in social work, public health, gerontology, Psychology, or related field from an accredited college or university; and one (1) year of the above-mentioned professional experience.
NOTE: "Professional experience" refers to work that is creative, analytical, evaluative, and interpretive; requires a range and depth of specialized knowledge of the profession's principles, concepts, theories, and practices; and is performed with the authority to act according to one's own judgment and make accurate and informed decisions.
LICENSE:
Appointees will be required to possess a driver's license valid in NJ only if the operation of a vehicle, rather than employee mobility, is necessary to perform essential duties of the position.

Knowledge of the purposes, policies, standards, and procedures involved in community relations and social service programs.
Knowledge of the programs and methods to be followed in coordinating such programs.
Knowledge of the organization, objectives, and activities of varied types of public and private social agencies.
Knowledge of the intricacies of group interaction and individual counselling.
Knowledge of the special needs and concerns of elderly people.
Ability to get along with people, respect them, and understand confidentiality and need for privacy.
Ability to organize assigned work and develop effective work methods.
Ability to plan and review programs involving the social needs of the elderly.
Good health and freedom from disabling physical and mental defects which would impair the proper performance of the required duties, or which might endanger the health and safety of oneself or others.
Ability to learn how to utilize various types of electronic and/or manual recording and information systems used by the agency, office, or related units.
Ability to read, write, speak, understand, and communicate in English sufficiently to perform duties of this position. American Sign Language or Braille may also be considered as acceptable forms of communication.
