COORDINATOR OF LONG TERM AND ALTERNATE CARE FOR THE ELDERLY
DEFINITION:
Under direction, plans, coordinates, and implements activities
involved in providing assistance to the elderly and their families in
identifying and making use of noninstitutionalized alternative care
services on a long term basis; does other related duties as required.

EXAMPLES OF WORK:
Maintains and updates an inventory of resources and services available
to the elderly, ascertains the adequacy of such programs and services,
and provides for alternatives as needed.
Gathers and disseminates information on the subject of the elderly.
Assists the elderly in acquiring services necessary to maintain an
adequate level of care in their home such as meals, health care
companionship, transportation, medical services, and so forth.
Serves as liaison with agencies providing services to the elderly in
the interest of resolving problems, improving their scope of services,
and facilitating provision of services to this target population.
Meets with the elderly and their families to discuss and evaluate
their circumstances, identifies and establishes the priorities of
their needs, and devises a plan to address them effectively
Provides information to the public and the elderly on the specialized
care available in their area of need and location, and on the
procedures to follow to benefit from them.
Prepares correspondence, reports, and informational articles.
Maintains records and files.
Visits facilities and agencies providing services to the elderly to
ascertain their services to their clients and to promote cooperation
and assistance in the provision of services to the elderly.
May recruit, train, and supervise activities of volunteers providing
services to senior citizens in the area of companionship, recreation,
basic nursing care, advocacy, and so forth.
Keeps informed on the latest developments on health and legislative
matters concerning the geriatric field and serves as a resource in
this area.
Gives talks and presentations to interested groups in the area of
geriatrics and related themes.
Plans, organizes, and leads seminars and/or workshops on matters
concerning the elderly.
Will be required to learn to utilize various types of electronic
and/or manual recording and information systems used by the agency,
office, or related units.
REQUIREMENTS:
EDUCATION:
Graduation from an accredited college or university with a
Bachelor's degree in the social, behavioral, or health care sciences
including or supplemented by 15 semester hour credits in the field of
gerontology.
EXPERIENCE:
Three (3) years of experience in work involving counseling or
assisting senior citizens with matters involving their welfare.
LICENSE:
Appointee will be required to possess a driver's license valid in
New Jersey only if the operation of a vehicle, rather than employee
mobility, is necessary to perform essential duties of the position.
KNOWLEDGE AND ABILITIES:
Knowledge of problems encountered by the elderly and alternatives
available to address them effectively.
Knowledge of the purposes, methods, and theories of geriatrics and
gerontology and their application to specific situations.
Knowledge of community agencies and state and federal programs
and resources available to the elderly, and the methods of applying
for them.
Ability to work with the public and established agencies to involve
them and to coordinate their efforts in developing community programs
for the elderly.
Ability to work with the elderly in ascertaining their conditions and
needs and in advising them of available programs in which they may
participate and from which they may benefit.
Ability to prepare clear, accurate, and informative reports containing
facts, conclusions, and recommendations.
Ability to analyze and evaluate information on case records for the
elderly to reach conclusions based on identified facts which will
promote their benefit and address the identified needs effectively.
Ability to learn to utilize various types of electronic and/or manual
recording and information systems used by the agency, office, or
related units.
Ability to read, write, speak, understand, or communicate in English
sufficiently to perform the duties of this position. American Sign
Language or Braille may also be considered as acceptable forms of
communication.
